Requirements

Experience

24 months of role experience is required with some in the last 12 months.

9 months of Social Work experience is required with some in the last 24 months.

3 months of experience with Meditech is preferred from any number of months.

References

Candidates must provide 2 references from within the past 12 months in order to be considered for this role

Additional Details:

Patient Ratios: SW if pair with Matrix of another RN will work Complex case of 8 or 9. If they are just covering DC planning then will have 1:25 or up to 30

Documentation System: Nate, Care Team Assignment

Call Requirements: All work every third weekend

Floating policy: All Social workers float-except trauma.

Patient Experience requirements: Knowledge of Hospital DC planning/execution and knowledge of MDR or IDT.

Must have:

· MIDAS/Meditech Experience, PASRR, Navihealth

· MSW or LCSW

Must have at least 9 months of same unit experience in the past 12 months, no exceptions.
