Requirements
Experience
12 months of role experience is required with some in the last 12 months.
Travel experience is required from any number of months.
Must have at least 22 months of Case Management experience.
Trauma Level 2 experience is preferred from any number of months
3 months of experience with Epic is required from any number of months.
References
Candidates must provide 2 manager/supervisor references from within the past 3 years in order to be considered for this role
License
Candidates must have a California license

Additional Details:

References must cover at least 1 full year of relevant experience in specialty within the past 3 years.

Cannot have any hits/suspensions on license or any background hits from any point in the past.

Required Skills/Experience: 2-year acute medical Care Manager experience performing Care Coordination. Acute Hospital, Case Management/Utilization Review, Appeals and Denials, Care coordination, Disability case management, Discharge Planning, Disease management, Needs Assessment/ Order DME, Plan of Care, Worker’s comp case management, Regulatory, CMS: Centers for Medicare and Medicaid Services, Department of Health, DRG (Diagnosis Related Groups), HIPAA guidelines (Health Insurance Portability and Accountability Act), OSHA: Occupational Safety and Health Administration, The Joint Commission/ Core Measure/National Safety Goals, Worker’s Compensation, and Acute Hospital Experience by Specialty, MS.

Preferred Skills/Experience: Emergency Department*, ICU*, and Surgical Services*.

Unit/Facility Details: All positions at Providence Health & Services require that any and all lab items requested during onboarding be completed within 72 hours of signing a contract - no exceptions. This means candidates must be responsive and willing to go to a lab to complete health orders ASAP after signing. Failure to comply may lead to a pushed start or cancellation as assessed by the facility.

Special Requests: RTO must be listed at time of submission. Pre-Approved Time Off: 1 Request. Anything more than 7 days RTO will potentially delay your offer while we run approval by the client. To minimize turn around please limit total RTO requests. Must commit to working 3 of the 5 holidays (Thanksgiving, Xmas Eve, Xmas Day, NYE, NYD)

Shift & Scheduling: Weekend Rotation. Weekend Coverage: Every Other. Number of Weekend Shifts Per Contract: varies to meet unit needs.

Floating Requirements: Floating Expectation: Float to the Laguna Facility. ALL travelers are expected to float between all facilities within a given state. travelers' refusal to float may result in termination of assignment and/or forfeiture of guaranteed hours.
