Requirements

Experience

12 months of role experience is required with some in the last 12 months.

Must have at least 12 months of Occupational Therapy experience within the past 3 years.

Experience with Oasis is required from any number of months.

Experience with Homecare Homebase (HCHB) is required from any number of months.

References

Candidates must provide 2 manager/supervisor references from within the past 2 years in order to be considered for this role

License

Additional Details:

Required skills and experience: Home Health experience required. Float experience does not count towards the unit experience requirement.

Shift & schedule requirements: Monday-Friday

Floating requirements: Coverage: Shasta Lake south to Orland May be asked to help in other territories within facility's service area as needed which is about a 60 mile radius from the Facility address. Average 4-6 visits per day depending on type of visit for PT/OT/SLP

Special request requirements: Any overtime must be pre-approved. Client may consider without Homecare Homebase if CLN agrees to complete online HCHB course prior to start date.
