Requirements

Experience

24 months of role experience is required with some in the last 12 months.

24 months of Home Health experience is required with some in the last 24 months.

Experience with Oasis is preferred from any number of months.

References

Candidates must provide 1 charge/lead reference from within the past 2 years in order to be considered for this role

Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role

License

Candidates must have a West Virginia license or compact license

Additional Details:

Previous perm staff must be gone for 2 years before they are eligible to return in a traveler capacity. No exceptions.

Artificial nails are not allowed for any travelers. Failure to adhere could result in being sent home and/or terminated if policy is not followed.

If candidate worked a perm facility in the last 12 months, 1 reference must be from perm facility.

LPNs only: Must have previously worked on at least (1) a travel assignment with Trusted and have a reference from that assignment

Roles requiring a state licensure only: Cannot have any licensure hits

Monday- Friday

Rotation of weekends and on call.

BSN Required: No

Common diagnosis / Types of patients/patient demographics: COPD, CHF, DM, mainly adults, rarely peds.

Additional Job Duties/Utilization Review involved? Admissions, use of Oasis

RT Equipment (vents, Cpap, BiPap, etc): Trilogy, trachs, oxygen, BiPAP occasionally

How many patients do they see per day? We see 5-6 patients daily for 8 hrs employees, depending on distance they travel for the day. 12 hr employees will be 8-9 depending on travel.

Shifts & Scheduling (Do you allow self-scheduling? Block scheduling?) Through week- M-F 8a-4:30p. Weekend position Saturday, Sunday, Monday 8a-8p

Weekend Requirements: None for weekend position, may have 1 every 6th weekend for day

Holiday Requirements: On rotational basis

Is there a call or standby requirement? No call

Scrub Color/Dress Code: Royal Blue Scrubs
