Unit/Facility Details: Proof of Identification - Drivers License/State ID, Current and valid DL or gov't issued photo ID

Special Requests: No Block Scheduling

Shift & Scheduling: Mon-Fri

Call Required: No

Weekend Requirement: No weekends

Floating Requirements: As needed per clinic needs
