Candidates with pending licenses accepted - License must be active prior to start. All therapy/rehab traveler start dates will be booked 6 weeks out to provide time to process Marshfield credentialing and onboarding requirements. All travelers will be granted temporary privileges through the Marshfield credentialing team. Marshfield's HR will reach out to the traveler directly alongside our Vaya compliance team throughout the onboarding process to collect these requirements.*

• The following documents are required to be submitted to compliance within 1 week of signing a contract. These requirements are needed urgently in order to process the Marshfield Clinic Health System credentialing application timely, and avoid onboarding delays.
• DOB
• SSN
• Resume
• Transcripts or diploma
• Education verification
• Minimum of 3 references (need name and email address for MCHS to send their reference form) - 2 must be peers, 1 must be the department chair
• If assigned to Beaver Dam, an Administrative reference must also be provided

Bachelor's degree in occupational therapy

Preferred: Master's or Doctorate degree in occupational therapy

Block Schedules: No

Clinician-to-Patient Ratios: 1:10

Patient Age Groups: Neonates, Adolescents, Infants, Adults, Pediatrics, Geriatrics

Daily Census: 220

Background initiation, BID, CCAP, and WI caregiver are to be completed and uploaded into Lotus within 48 hours

Current Driver's License or Government issued photo ID required.
