Resume: Each employment history entry must include all the following: detailed duties, hospital size, unit size, trauma level (if applicable), patient ratio, teaching facility. Any employment gaps greater than 30 days must be explained.

Shift & Scheduling: M-F, day shift; no call or weekends; occasionally may be the only Occupational Therapist at a location in LTC – phone support is available at all times

Required Skills/Experience: Experience with inpatient. Experience with SNF.

Preferred Skills/Experience: Experience in the acute care setting preferred; 1 year of experience in the past 3 years preferred but not required

Procedures/Diagnostics/Treatments Performed on Unit: Diagnoses include general deconditioning, Total Joint replacement, post op ortho; some neuro diagnoses; general medical-surgical; Coverage support for SNF facilities at Neilson Place and Blackduck MN providing coverage to facility residents requiring skilled therapy services.

Equipment Used on Unit: EPIC for electronic medical record (Acute Care); Casamba/NetHealth or LTC environments

Floating Requirements: Will be combined inpatient and LTC support needs

Unit Details: Hospital licensed for 118 but therapy caseload typically averages approximately 30-40 patients; Neilson Place is a 60 bed SNF, Blackduck GSS a 30 bed SNF.

Previous Employment Requirements: MUST note in submission if clinician has ever worked for Sanford Health. If clinician previously worked with Sanford, they must have an end date 6 months prior to their submission date.
