Requirements

Experience

24 months of role experience is required with some in the last 12 months.

Must have at least 24 months of Home Health experience within the past 4 years.

Experience with Oasis is required from any number of months.

Experience with Cerner is preferred from any number of months.

References

Candidates must provide 1 reference from within the past 3 years in order to be considered for this role

Candidates must provide 1 charge/lead reference from within the past 12 months in order to be considered for this role

License

BLS (Required)

Vaccinations

Required Skills/Experience:

Must also have one year experience working PT/OT/SP.

Preferred Skills/Experience:

Home setting experience is preferred but not required.

Shift & Scheduling:

Weekend Requirement: every 3rd weekend
Holidays: minor holidays, major only if needed

Unit/Facility Details:

PT, OT, Speech - Marian & Arroyo Grande (home based care)
They will be assigned to a geographical region

Procedures/Diagnostics/Treatments Performed on Unit and Types of Patients on Uni -PT/OT - ortho, strokes, ortho, joints, spinal injuries speech - anything

Eligibility as Current/Previous Employee: Cannot have worked at any CommonSpirit location within the last 12 months.
