Requirements

Experience

24 months of role experience is required with some in the last 12 months.

36 months of Case Management experience is required from the last 4 years.

Experience with Epic is preferred from any number of months.

References

Candidates must provide 1 charge/lead reference from within the past 2 years in order to be considered for this role

Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role

License

Candidates must have a West Virginia license or compact license

Certifications

BLS (Required)

Vaccinations

COVID vaccination required. Religious and medical declinations accepted.

Additional Details:

Previous perm staff must be gone for 2 years before they are eligible to return in a traveler capacity. No exceptions.

Artificial nails are not allowed for any travelers. Failure to adhere could result in being sent home and/or terminated if policy is not followed.

If candidate worked a perm facility in the last 12 months, 1 reference must be from perm facility.

LPNs only: Must have previously worked on at least (1) a travel assignment with Trusted and have a reference from that assignment

Roles requiring a state licensure only: Cannot have any licensure hits.

Case load # patients per day/consults per day: 20

𝗣𝗿𝗼𝗰𝗲𝗱𝘂𝗿𝗲𝘀/𝗗𝗶𝗮𝗴𝗻𝗼𝘀𝘁𝗶𝗰𝘀/𝗧𝗿𝗲𝗮𝘁𝗺𝗲𝗻𝘁𝘀 𝗽𝗲𝗿𝗳𝗼𝗿𝗺𝗲𝗱 𝗼𝗻 𝘁𝗵𝗲 𝘂𝗻𝗶𝘁:

What areas do they cover Inpatient, Outpatient, ER, Home Care, etc: Inpatient Only.

Common types of patients & job duties: Cardiac, Trauma, Oncology, Medsurg.

Required Skills/Experience: Discharge Planning.

Must have inpatient case manager experience and discharge planning.

Shift & Scheduling:

Monday through Friday.
