12 months of role experience is required with some in the last 12 months.

3 months of experience with Epic is preferred from any number of months.

References

Candidates must provide 1 manager/supervisor reference from any number of months in order to be considered for this role

Integrated Case Manager

Required Skills/Experience: Minimum 5 years acute inpatient case management experience with experience in acute discharge planning

Previous experience using CarePort system.

Preferred Skills/Experience: Candidates with previous experience charting in AllScripts, GEMS/Sunrise are preferred.

Required Credentials: Nursing degree or MSW

MLSW certification

Registered Nurse (RN) with a valid, unrestricted State of Michigan license or Licensed Social Worker (LMSW) with a valid, unrestricted State of Michigan license.

Shift & Scheduling: Weekend rotation required.
