Requirements

Experience

12 months of role experience is required with some in the last 12 months.

12 months of Physical Therapy experience is preferred from last 12 months.

Experience with Netsmart Technologies is preferred from any number of months.

References

Candidates must provide 1 charge/lead reference from within the past 3 years in order to be considered for this role

Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role

Float Requirements: occasional travel to other branches as needed

Mileage reimbursed at government approved rate

Weekend/Holiday: PT's rotate one Saturday every 6-8 weeks. (the Sat. they work would be given a day off during the week) Saturday only. No holiday.

Required Education: Bachelors degree

Minimum Experience: Home Health experience, training working with the adult/geriatric population with a variety of orthopedic, neurological and functional mobility diagnosis.

Knowledge of Physical Therapy standards and practices acquired through a minimum of a Masters degree in Physical Therapy.

Charting/Equipment: Laptop for point of service documentation and cell phone for GPS and communication.

Population served includes adult and geriatric patients.

Meets productivity expectations of an average of 4.5 visits per day
