Previous Employment Requirements: Former Perm Employees: must be separated for a minimum of 6 months before being eligible to return as a traveler. Must be approved by HR

Preferred Skills/Experience: Proficiency in HCHB (Homecare Homebase) preferred

Types of Patients on Unit: No pediatric patients. Adults only. Neuro, cardiac, respiratory, deconditioned from surgeries and conditions, ortho, post-surgical, post fall, cancer

Unit/Facility Details: Parking Cost: Free

Procedures/Diagnostics/Treatments Performed on Unit: Auto insurance, driver's license, ability to drive in snow, rain and slush and on gravel roads. Ability to drive 30 miles/day on average and up to 100 miles on rare occasions. Ability to use iphone and navigation app. Ability to create a patient schedule.Flexible/adaptable. Use outlook for email and calendar appts. Use Teams for meetings. Order PPE and therapy supplies via Medline online to be drop shipped to their home or patient's home.

Equipment Used on Unit: Preferred: Wheelchair evaluation, equipment acquisition, contact APS, sync docum. daily, rover/mobile Epic, transfer pole, power w/c mobility, manual w/c mobility

Special Requests: Travel Pairs: accepted, however each traveler will be assigned to their own territory and must have own transportation

Shift & Scheduling: On-call rotation is required, Must be willing to work 4 weekdays and 1 weekend day each week (Tu-Sat) or (Sun-Th + 3 weekdays) 8 hr shifts only.
