Requirements
Experience
12 months of role experience is required with some in the last 12 months.
24 months of Physical Therapy experience is required with some in the last 24 months.
Experience with PointClickCare is preferred from any number of months.
References
Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role
Candidates must provide 1 reference from within the past 3 years in order to be considered for this role
License
Candidates must have a license
Certifications
BLS (Required)
Vaccinations
COVID vaccination required. Religious and medical declinations accepted.

Additional Details:
Dementia testing will be required prior to start.
1 year of experience must be within a Skilled Nursing Facility.
8 h days, need to cover Home Health and SNF. Radius of 40 miles, 5 cases per day
No on call and call back available or billable at GSS.
Settings: home care, hospice, and long term care
Must have home health experience, no on site supervisor.
Shift: Monday thru Friday
