Requirements

Experience

12 months of role experience is required with some in the last 12 months.

Must have at least 12 months of Home Health experience with 12 months within the past 3 years.

Experience with Homecare Homebase (HCHB) is required from any number of months.

Experience with Oasis is required from any number of months.

References

Candidates must provide 2 manager/supervisor references from within the past 12 months in order to be considered for this role

License

Additional Details:

Shift & Scheduling: M-F

Required Skills/Experience: 1 YR of recent Skilled Home Health Experience (at least 1 year within the last 3 years). Oasis experience is required for all req’s requiring start of care.

Types of Patients on Unit: Case Load: Geriatric. System does a good job grouping clients minimizing drive and windshield time. Occasional maximum radius of 50+ miles.

Special Requests: Traveler is expected to have their own transportation. System does a good job grouping clients minimizing drive and windshield time. Occasional maximum radius of 50+ miles.
