Requirements

Experience

36 months of role experience is required with some in the last 12 months.

12 months of travel experience is required from any number of months.

Must have at least 12 months of Case Management experience.

References

Candidates must provide 2 references from within the past 2 years in order to be considered for this role

Additional Details:

Cannot currently be employed anywhere else on a per-diem basis

Will work in care management dept.

Shift: Monday to Friday; every other weekend.

Required hospital discharge planning experience (3 years) for adult, pediatric and maternal health patients.

Must complete admission assessments, create discharge plans involving home care, subacute and acute rehab patients, work with homeless patients, difficult complex discharges.

Will either work in one unit primarily or will rotate various units. Will attend discharge daily planning meets, work with other staff in creating discharge plans. Will talk to families and patients frequently. This is not a therapy role.
