Requirements
Experience
12 months of role experience is required with some in the last 12 months.
12 months of Hospice experience is required with some in the last 24 months.
3 months of experience with Homecare Homebase (HCHB) is required from any number of months.
References
Candidates must provide 2 manager/supervisor references from within the past 12 months in order to be considered for this role
License
Candidates must have a Missouri license or compact license
Certifications
BLS (Required)
Proof of Identification
Driver's license required

Additional Details:
EMR: HCHB Experience Required
Requirements/Experience: **Must have at least 12 months of Hospice experience within the past 3 years
Territory: 60 mins from Columbia office to the East and West. Approx 20 min to the South and 30 min to the North.
Any call?: LPN’s rotate back-up call and take about 4 days a month with at least 1 weekend day/nigh
