Requirements

Experience

12 months of role experience is required from last 3 months.

3 months of travel experience is preferred from any number of months.

12 months of Case Management experience is required from last 3 years.

3 months of experience with Epic is preferred from any number of months.

References

Candidates must provide 2 manager/supervisor references from within the past 3 years in order to be considered for this role

License

Candidates must have a Kentucky license or compact license

Position Duties Discharge planning

Preferred Qualification: Care Port/NaviHealth

Monday-Friday

1 weekend per 12 weeks

Required Skills: These nurses will be doing discharge planning, and will NOT be responsible for any utilization review/utilization management. They will be working in Epic (EHR) and CarePort/NaviHealth (referral system). Experience in both of these systems is strongly preferred. RN will be responsible for all discharge arrangements, such as outpatient IV antibiotics, wound vacs, home care, rehab, etc.
