Experience

12 months of role experience is required with some in the last 12 months.

Must have at least 12 months of Case Management experience within the past 12 months.

References

Candidates must provide 1 charge/lead reference from within the past 2 years in order to be considered for this role

License

Candidates must have a California license

Additional Details:

Required Skills/Experience: Knowledge of post-acute care, care coordination, case management, business development/marketing

Preferred Skills/Experience: Understanding of insurance authorizations and process

Shift & Scheduling: Weekends On Request • Call Requirements: None • Shift: Monday through Friday with 1 weekend a month. The weekend shift is 4 hours Sat and 4 hours Sunday (they then flex off the following Thursday).

Floating Requirements: Clinical liaisons often “float” to cover different facilities. So some local travel to a neighboring hospital may occur.
