Requirements

Experience

12 months of role experience is required with some in the last 12 months.

Must have at least 36 months of Case Management experience.

References

Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role

License

Candidates must have a Maine license or compact license

Certifications

BLS (Required)

CCM (Preferred)

Vaccinations

Additional Details:

Care Manager

Shift & Scheduling: Variable 8 - 4:30 pm or 4 - 10 hour shifts of 7:30 am to 5:00 pm

No Call

Weekend and Holiday Rotation is required

Required Skills/Experience: Must have utilization review and discharge planning experience.

3-5 years clinical experience in medical/surgical nursing, home care, hospital case management and/or previous utilization review experience

Required Credentials: Associate or Bachelor degree-prepared Registered Nurse

Preferred Credentials: Certification by a nationally recognized case management or utilization review organization preferred

CCM or ACM preferred.
