Additional Details:
Required Skills/Experience: 1 year of skilled home health experience.
Required Credentials: Cannot have any hits/suspensions on license or any background hits from any point in the past.
Special Requests: RTO must be listed at time of submission. All positions at Providence Health & Services require that any and all lab items requested during onboarding be completed within 72 hours of signing a contract. Candidates must be responsive and willing to go to a lab to complete health orders as soon as possible after signing. Failure to comply may lead to a pushed start or cancellation as assessed by the facility. References must cover at least 1 full year of relevant experience in specialty within the past 3 years. You must be able to complete labs within 48 hours of signing. Cannot be currently or recently (within 6 months) employed with the network; former employees must have left in good status. ER profiles must include the size of the facility / # beds and the trauma level designation, if any.
Unit Details: Adult and geriatric patient population. May visit up to 6 patients per day; 30 productivity points standard. Requires own transportation. System groups clients to minimize drive and windshield time; occasional maximum radius of roughly 60 miles (60-90 minute total drive time) per day. Uses Rover phone app.
Shift & Scheduling: No weekends. No call. Orientation for all Oregon facilities is in Portland, OR for RN, LPN and CNAs.
