Additional Details:
All positions at Providence Health & Services require that any and all lab items requested during onboarding be completed within 72 hours of signing a contract - no exceptions. This means candidates must be responsive and willing to go to a lab to complete health orders ASAP after signing. Failure to comply may lead to a pushed start or cancellation as assessed by the facility.
References must cover at least 1 full year of relevant experience in specialty within the past 3 years.
You must be able to complete labs within 48 hours of signing.
ER profiles must include the size of the facility / # beds and the Trauma Level designation (if any).
Orientation for all Oregon facilities is in Portland, OR for RN, LPN and CNAs.
Required Skills/Experience:
- 1 year of skilled home health experience.
Required Credentials:
- OR and WA licensure required
- Cannot have any hits/suspensions on license or any background hits from any point in the past.
Special Requests:
- Traveler is expected to have their own transportation.
- RTO must be listed at time of submission.
- RTO restrictions: Confirm with hiring manager.
- Cannot be currently or recently (within 6 months) employed with network; must have left in good status if former employee.
Unit Details:
- Covers East Portland, OR, Clark County, WA, and surrounding areas.
- 1:1 ratio; may visit up to 6 patients per day; 30 productivity points standard.
- System groups clients to minimize drive and windshield time. Occasional maximum radius of roughly 60 miles (60-90 minute total drive time) per day.
- Uses Rover (phone app).
- Free parking
Shift & Scheduling:
- No weekends.
- No call.
