Required Skills/Experience: 2 years recent Wound Care RN or Wound Care Coordinator experience.
Unit Details: Manages the facility wound care program, including standards of care and practice related to wound, ostomy, and continence patient care needs. Provides direct patient care, assists staff nurses, and serves as a consultative service to affiliated healthcare agencies. Develops and implements patient care protocols, documentation tools, wound care formulary, and WCC referral criteria. Establishes standards of care, competencies, policies, and procedures for wound care; serves as a primary consultant to wound care clinicians, facility staff, and physicians; and provides ongoing staff education on wound care products and protocols. Evaluates wounds on admission and ongoing, consults on pressure injuries and wounds not showing measurable signs of healing within two weeks, and assists with comprehensive wound management and prevention plans of care. Reviews wound-related medical records and documentation, recommends changes to wound care orders with evidence-based research support as needed, investigates wound-related adverse events through root cause analysis, participates in clinical outcome monitoring and performance improvement initiatives, completes wound-related LTRAX data set records for CMS quality reporting, and assists with training new hire Wound Care Coordinators.
