Must note in submission if clinician has ever worked for Marshfield; previous Marshfield employment must have an end date 6 months prior to submission.
Required Skills/Experience: Chronic disease management, patient/family education, NPWT, ostomy care, central line care, IV administration, complex wound care, and hospice experience with pain and symptom management and end-of-life care.
Unit Details: Home care and hospice for primarily geriatric patients with occasional pediatrics; common cases include heart disease, chronic respiratory diseases, CHF, COPD, pneumonia, dementia/Alzheimer's, depression, stroke, cancer, kidney disease, diabetes, liver disease, wounds, joint replacement, surgery, medication management, and catheter management. Will do both home health and hospice visits. Weekly IDT meeting and case conference at office; monthly all staff meeting. No vents. CPAP and BiPAP on occasion; patients self-manage. Will see 5-6 patients per day depending on type of visit, 25-30 points per week; routine and OASIS visits.
Shift & Scheduling: Monday through Friday; end time may vary. Weekends only required during on-call shift. On-call 2 times per week, Monday-Thursday.
