Additional Information
In addition to a valid driver's license, you MUST include a proof of address document to apply. The document must be in your name & match the address on your driver's license (and issued within the last 3 months). Any document other than those listed below will NOT be accepted:
• Cell Phone Bill
• Electric, Gas, Water, Sewer, or Wi-Fi
• Mortgage Statement or Lease Agreement
You must be able to complete labs within 48 hours of signing.

No holiday RTO requests will be accepted.
Unit Details:
Adult and geriatric patient population.
Patient population includes bariatric surgery, stroke, acute respiratory failure, fall, aneurysm, and encephalopathy.
Shift & Scheduling:
Weekend coverage: Every other weekend.
No more than 5 days of RTO will be accepted.
Floating Requirements:
Floating to sister facilities is required.
