Home health visit productivity points for PT: death at home 0.5; discharge without visit 0.5; follow-up 2; initial evaluation 1.5; lab draw 0.5; Oasis discharge 1.5; preceptor day NN 6; preceptor day other 5.6; reassessment 1.5; recertification 2; regular visit 1; resumption of care 2; ROC in last 5 days 2; start of care 2.75; supervisory therapy 1.
Special Requests:
You are not allowed to work for both Riverside Travel assignments and Riverside Per Diem contracts simultaneously. You must choose one or the other.
