Travel Nurse (RN) - Case Management

References: At least one reference must be from the same unit/specialty as the job you're applying to.

Required Skills/Experience:

• Minimum 2 years of acute inpatient medical case management experience that includes at least 1 year (in the last 3) in a large busy Hospital ED (500+ beds, Level 1/academic medical center).
• Recent (within the last two years) acute care discharge planning experience for patients with a wide range of medical diagnoses is required.
• Heavy telephonic, utilization management/review, insurance company, rehab, LTC, or hospice experience is not the experience sought.
• Previous travel in a Level 1 facility within the last 3 years.

Unit Details:

• Caseload of 15-20 patients/consults per day.
• Works alongside the case management and social work teams; not working alone.
• Handles complex placements; local facility contracts support uninsured or underinsured patients when criteria are met.
• Variety of complex patients, including patients with no other hospital options.
• Consults with respiratory therapy and pharmacy as needed for discharge planning.

Shift & Scheduling:

• Mon-Fri.
• No weekends.
• Holidays rotated as needed.

Floating Requirements:

• Covers all inpatient units throughout the hospital.
