Requirements

Experience

12 months of role experience is required with some in the last 12 months.

Experience with Oasis is preferred from any number of months.

References

Candidates must provide 1 charge/lead reference from within the past 12 months in order to be considered for this role

Candidates must provide 1 manager/supervisor reference from within the past 3 years in order to be considered for this role

Resume: Each employment history entry must include all the following: detailed duties, hospital size, unit size, trauma level (if applicable), patient ratio, teaching facility.

Preferred Skills/Experience: Would prefer someone with Home Health experience.

Required Credentials: Must have current Driver's License & Auto Insurance.

Unit Details:

Do they cover both Home Health and Hospice? Majority Home Health, may be asked for a safety evaluation or teaching 1 time visit in Hospice situation.
Additional Information: We do all ages from pediatrics to adults however peds are few and far between.

Previous Employment Requirements: Candidates may not be full time, part time, or PRN with any facility associated with CommonSpirit, including CHI and/or Dignity within the past 12 months.

Shift & Scheduling: Monday through Friday, 8-5. Holidays and weekends off.

Eligibility as Current/Previous Employee: Cannot have worked at any CommonSpirit location within the last 12 months.
