Required Skills/Experience:

• Current in-patient case management experience.

Unit Details:

• Coordinates care for a select caseload from triage through care coordination during JHM management and safe transfer home.
• Proactively communicates with payors to ensure approval of treatment plans and required resources.
• Maintains and responds to clinical system and fiscal data related to specific patient populations.
• Collaborates with the multidisciplinary team and patient/family to support effective and efficient transition planning.
• Coordinates transition to post-acute settings including nursing facilities, acute inpatient rehab, and local shelters.
• Enters and maintains patient information in departmental systems per operational guidelines.
• Will not have access to or administer medications to patients.
• Uses NaviHealth for post-acute facility referrals.

Shift & Scheduling:

• No weekends, no holidays, no call.
