Unit Details:

• Evaluation and treatment of short-term and long-term patients in the SNF setting.
• Works with supervision and assistance of COTAs while completing timely documentation and treatments.
• Develops individualized treatment plans based on physician orders to improve independence with ADLs, functional mobility, cognition, upper extremity function, and overall quality of life.
• Collaborates with the interdisciplinary team to promote safe discharge and maximize residents' functional potential through various treatment approaches.

Shift & Scheduling:

• 8 hours/5 days a week (flexible).
