Required Skills/Experience:

• Home health experience including OASIS required.

Required Credentials:

• NPI number required.
• Proof of automobile insurance required.

Special Requests:

• Cannot have been directly employed full-time, part-time, or PRN by any CommonSpirit, CHI, or Dignity facility within the last 12 months; PRN employment through outside firms will be considered.
• Cannot apply to more than one job order on the same unit and shift at the facility.
• Holiday work is required, but time-off requests may be considered.

Unit Details:

• All treatments are provided in homes, assisted living facilities, senior apartments, and board and care homes.
• 3 to 5 patients per day depending on the visit; 2 Start of Care visits per day, or 3-4 evaluations, or any combination to make 3 to 5. Visits are weighted to cover travel, documentation, and case management; a Start of Care is weighted at 3.75 to 4 hours total.
• Varied diagnoses; no pediatrics or hospice. Treats ortho, cardio, pulmonary, oncology, neuro, geriatric, and amputations.
• Must be able to work in home environments that may be cramped, hot/cold, or cluttered/bug-infested.
• Supervises up to 2 PTA/COTAs.

Shift & Scheduling:

• Mon-Fri.
• Work is expected to be completed by the end of the shift.
• 1 weekend per month requested but not required.

Floating Requirements:

• Area depends on unit needs. There are 3 teams that service the entire service territory. Patients are seen from all Mercy area hospitals and Mercy Community doctors, primarily serving Mercy San Juan Hospital in Sacramento. Territories are grouped to decrease excessive mileage and drive time, but clinicians go where the patients are and may cover patients on a different team as needed. Woodland services Yolo County and has 2 service teams.
