Required Skills/Experience:

• 1 year Certified Home Care experience and 1 year Case Management experience required. Experience in hospice, private care, or working as a Case Manager on its own does not meet the qualifications for Certified Home Health Care Management experience. Only roles specifically within a certified home health agency will be considered relevant for the 1-year requirement. Ability to travel to cases as assigned.

Required Credentials:

• Contingent/temp licensure is not accepted.
• Primary source verification of licensure/certification is required for application.

Unit Details:

• Provides physical therapy services in patients' residences for patients from birth to over 100 years old with physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes.

Shift & Scheduling:

• Weekend rotations are once every 6 weeks.

Floating Requirements:

• Travel to cases as assigned throughout Andover, Bedford, Billerica, Burlington, Concord, Hanscom, Lawrence, Lexington, Lincoln, Nutting Lake, Pinehurst, Reading, Stoneham, Tewksbury, Wakefield, Wilmington, Winchester, and Woburn.
