Required Skills/Experience: 1 year Certified Home Care experience required. 1 year Case Management experience required. Experience in hospice, private care, or working as a Case Manager on its own does not meet the qualifications for Certified Home Health Care Management experience. Only roles specifically within a certified home health agency will be considered relevant for the 1-year requirement.

Required Credentials: Contingent/temp licensure is not accepted. Primary Source Verification of licensure/certification is required for application.

Unit Details: Provide physical therapy services to patients in their place of residence who have physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes. Patients may range in age from birth to over 100 years. Coordinate services with patients, families, physicians, and other allied health professionals to identify patient needs and meet those needs. Utilize electronic documentation and interdisciplinary communication in accordance with agency policies, procedures, and standards of practice. Coverage area includes Boxford, Danvers, Hamilton, Lynnfield, Middleton, North Andover, North Reading, Peabody, Topsfield, and Wenham. Travel to assigned patient cases.

Shift & Scheduling: Weekend rotations are once every 6 weeks.
