Additional Details:
Required Credentials: Must use a personal automobile and maintain a valid driver's license, auto liability/property damage insurance as required by law, and a vehicle in good working order.
Preferred Skills/Experience: Home health or case management experience preferred.
Special Requests: Previously employed clinicians may be considered only as a one-off exception and must submit a written request to apply.
Unit Details: Supports patients and family members through the final stages of illness in home settings, including patients' residences, skilled nursing facilities, assisted living facilities, and adult foster homes. Serves patients in Linn, Benton, Marion, Polk, Lincoln, and Tillamook counties. Services include intermittent skilled nursing visits, pain and symptom management, medical social services, counseling, spiritual support, hospice aides, trained hospice volunteers, bereavement support, and medications/equipment as appropriate.
Shift & Scheduling: Rotating weekends. Holidays as required. 24-hour on-call nursing services available. 40 contract hours per week.
