Additional Details:

All positions at Providence Health & Services require that any and all lab items requested during onboarding be completed within 72 hours of signing a contract - no exceptions. This means candidates must be responsive and willing to go to a lab to complete health orders ASAP after signing. Failure to comply may lead to a pushed start or cancellation as assessed by the facility.

References must cover at least 1 full year of relevant experience in specialty within the past 3 years.

You must be able to complete labs within 48 hours of signing.

Required Skills/Experience:

• 1 year of Skilled Home Health Experience.

Preferred Skills/Experience:

• National Patient Safety Goals/Core Measures preferred.
• Billing preferred.
• Acute disorder experience preferred.
• Chronic disorder experience preferred.
• Functional activities experience preferred.

Required Credentials:

• Cannot have any hits/suspensions on license or any background hits from any point in the past.

Special Requests:

• RTO must be listed at time of submission.
• Cannot be currently or recently (within 6 months) employed with the network. Must have left in good status if a former employee.
• Traveler is expected to have their own transportation.

Unit Details:

• May visit up to 6 patients per day; 30 productivity points standard.
• Visits are grouped to minimize drive time; occasional maximum radius is roughly 60 miles (60-90 minute total drive time) per day.
• Adult and geriatric patient population.
• Patient population includes dementia, Parkinson's, ALS, MS, ortho post-op, and post-stroke patients.
• Uses Rover phone app for charting.

Shift & Scheduling:

• 5x8 schedule only; 4x10s and 3x12s cannot be accommodated.
• No weekends.
• No call.

Floating Requirements: West Portland OR and Surrounding Areas

Details:

• AUTO OFFER - NO INTERVIEW: This is an auto-offer position. You will likely not interview with the facility prior to receiving an offer. However, an interview can be arranged upon request. All time off requests must be submitted at application. Scheduling requests must be worked out with the manager directly upon starting the assignment. Any offers for this job will require a clear "Yes" or "No" response from the nurse within 24hrs or the offer must be declined.
