Travel Allied (Physical Therapist) - Home Health

Required Skills/Experience:
1 year of recent skilled home health experience (at least 1 year within the last 3 years).
OASIS experience is required for start of care.

Special Requests:
Traveler is expected to have their own transportation.

Unit Details:
Geriatric caseload.
Visits are grouped to minimize drive time.
May visit up to 6 patients per day; 30 productivity points standard.

Shift & Scheduling:
Mon-Fri.
