Required Skills/Experience:

• Must have some home health experience.
• Must have 1 year post grad PT experience.
• Must have Ortho experience.

Preferred Skills/Experience:

• Prefer post grad home health experience.

Special Requests:

• Any employment gaps greater than 30 days must be explained.
• The facility may make one-off exceptions if previously employed at the facility, but exceptions must be approved by St. Luke's Health System.

Unit Details:

• In-home therapy with admissions, existing patients, and recertifications; includes a few assisted living patients.
• Multidisciplinary adult home health caseload; approximately 78% Medicare patients, primarily elderly, rehab, CVA, and inpatient surgical referrals including ortho, surgical, and wound patients; no lymphedema.
• Limited oxygen requirements.
• 4-6 patients per day.
• Can start shift from home and do not need to stop in an office.
• Coverage area up to 60 miles east and west, about 60 miles north to Sun Valley, and about 40 miles south to the Idaho/Nevada border.
• Monday and Wednesday multidisciplinary meetings to discuss current admissions.

Shift & Scheduling:

• Mon-Fri.
• No weekend requirements.
• No holiday requirements.
