Travel Nurse (RN) - Case Management

Required Skills/Experience:
2+ years of current home care/discharge planning experience or case management.

Unit Details:
The role is performed on site.
Provides discharge planning support for patients transitioning to home-based services and facilitates continuity of care.
Acts as a discharge planning resource for hospital staff involved in home care planning.
Verifies that patients have the devices needed for nursing and home care, such as walkers and commodes.
Sets up home care services when needed and advises on appropriate support organizations in the community.
Teaches patients and their families.
The role involves a thorough understanding of available home care services and products, eligibility requirements, and community resources.
Serves as an on-site home-based care expert.

Shift & Scheduling:
The schedule is Monday-Friday, with weekend coverage potentially needed; the employee must also be available for Saturday and Sunday hours of 8:30 am-4 pm.
