Preferred Skills/Experience:
Acute hospital case management experience preferred.
Special Requests:
Parking is available anywhere except the ER parking area.
Unit Details:
Case manager ratios are 1:25 on floors and 1:20 in UC units.
Uses an iPad, computer, and cell phone.
Case management uses Midas, InterQual, and CarePort; Midas is used for documentation and CarePort for referrals.
Responsibilities include assessments and evaluations.
Provides discharge planning with patients and families.
Shift & Scheduling:
The traveler is expected to work every other weekend and holidays in rotation with hospital employees.
