At least one reference must be from the same unit/specialty as the job you're applying to. Required Skills/Experience:
- 1 year of experience within the past 3 years must be within a Skilled Nursing Facility.
Preferred Skills/Experience:
- Prior work experience in a rehabilitation/geriatrics setting preferred.
Required Credentials:
- Dementia testing will be required prior to start.
Special Requests:
- Must take a full 6 months off from any Sanford Health and Marshfield Clinic Health System location.
- Must disclose previous employment within the submission for a DNU check.
- Any employment gaps greater than 30 days must be explained.
Unit Details:
- Supervises 2 COTAs.
- Uses Net Health and HCHB.
- Productivity expectations: SNF 82% (evaluation, group, and concurrent time is counted in full); Home Health 90%, including drive time and documentation time.
- Onsite therapy supervisor.
- Administers therapeutic interventions, assesses client needs, develops therapy plans, documents care, consults with physicians and interdisciplinary team members, trains clients/family members/staff on interventions and equipment, and may supervise therapy assistants.
Shift & Scheduling:
- Mon-Fri or able to work a shifted schedule Tue-Sat or Sun-Thu.
- Flexible contract hours: 40 or 36 per week.
Floating Requirements:
- May work in SNF and Home Health settings, including Grand Island SNF, Hastings SNF, and home health in the surrounding areas.
