Required Skills/Experience:
One year of relevant education may be substituted for one year of required work experience, or one year of relevant professional-level work experience may be substituted for one year of required education.
Required Credentials:
High School Diploma/GED or equivalent.
Bachelor's degree in Physical Therapy earned prior to January 1, 2015, from an accredited program, may be accepted based on historical requirements.
Master's degree in Physical Therapy earned prior to January 1, 2015, from an accredited program, may be accepted based on historical requirements.
Doctor of Physical Therapy (DPT) degree from an accredited program.
Unit Details:
Conducts discharge planning early in the treatment process, involving the patient, caregiver, and other team members, and completes the discharge summary.
Shift & Scheduling:
Monday-Friday.
