Unit Details:
ED Holding is expected to be the primary work area. It is a fast-paced unit with mixed diagnoses, isolation patients, and non-ambulatory patients.
ED Holding assignments are 1:2 for ICU patients, 1:4 for PCU patients, and 1:5 for med-surg patients; ICU and PCU nurses have a 1:5 assignment when caring for med-surg patients.
Elsewhere, ratios are 1:2 in ICU based on acuity, 1:4 in PCU and CCU depending on acuity or whether a patient is discharging, 1:5 in medical and surgical units, and 1:8 in rehab under a team model (RN, LPN, and tech).
ICU cases may include respiratory failure, GI bleeds, DKA, sepsis, complicated postoperative patients, CVAs, flap surgeries, post-arrest patients, ETOH, and IVDA.
Higher-level therapies include Rotoprone, manual proning, hypothermia protocol, CRRT, TPA, and flap surgeries, but these patients will not be taken.
No craniotomies, major traumas, or major cardiothoracic cases.
PCU patients may include respiratory compromise, sepsis, renal failure, bleeding disorders, general medical-surgical patients, CHF, COPD, pneumonia, general cases, and orthopedic cases.
Med-surg assignments may include oncology patients who are neutropenic or postoperative, psychiatric patients, and general medical-surgical patients with postoperative needs, COPD, CHF, sepsis, DVT, AKI, dialysis, and COVID; COVID is common on this floor.
Med-surg orthopedic assignments include total joints, postoperative patients, and general medical overflow.
Med-surg palliative assignments may include pneumonia, sepsis, CHF, COPD, AKI, DVT, PE, occasional surgical cases, and COVID.
Rehab includes neuro patients with stroke, spinal cord injury, TBI, multi-trauma, post-COVID, and post-ICU conditions; ventilated and tracheostomy patients are present. Patient acuity is generally med-surg level, and respiratory therapy provides just-in-time training.
The transport RN transports patients throughout the hospital, including all inpatient units, ED, CT, MRI, nuclear medicine, PACU, and Cath Lab. After 2300, when transports slow, the RN supports ED Holds with breaks and medication administration when not transporting.
Uses Alaris IV pumps and Pyxis medication dispensing.

Shift & Scheduling:
Must call the RMC staffing office at least 30-45 minutes before each shift to get the assignment; this is the preferred option.
The assignment is also posted outside the door of the House Supervisor's office.
Weekends vary depending on the needs of the unit.
Schedules are provided 3 weeks at a time and received around 10 days before the schedule begins.
Self-scheduling is not available.
No call.

Floating Requirements:
Floating is primarily to ED Holding and supports GSH 1 Dally/2 River North and South/Float Pool teams.
Floating may occur for partial shifts.
Floating will be within scope of practice and below.
Coverage may also include COVID overflow.
If PCU staff float to MS, the patient ratio is 1:5.
