Required Skills/Experience: 1 year case management experience required. 1 year Certified Home Care experience required. Experience in hospice, private care, or working as a case manager on its own does not meet the qualifications for Certified Home Health Care Management experience. Only roles specifically within a certified home health agency will be considered relevant for the 1-year requirement. Ability to travel to cases as assigned.

Required Credentials: Contingent/temp licensure is not accepted. Primary Source Verification of licensure/certification is required for application.

Unit Details: Provides physical therapy services in patients' residences. Patient population ranges in age from birth to over 100.

Shift & Scheduling: Weekend rotations are once every 6 weeks.

Floating Requirements: Coverage area includes Back Bay, Beacon Hill, Boston, Cambridge, Charlestown, Chinatown, Fenway, Financial District, Kenmore, North End, Prudential, South Boston, South End, and West End.
