Required Skills/Experience:
At least one (1) year of utilization review, case management, or discharge planning experience in an acute care hospital is required unless the posting specifies a training position.
Progressive Care, Critical Care and/or ER experience is required.
Procedural sedation experience is required.
Airway management is required.
EKG interpretation is required.
Experience with moderate sedation is required.
Experience with sedatives and reversal medication is required.
Phase II recovery experience is required.
Recent charge nurse experience is required.
Required Credentials:
Associate's Degree in Nursing.
Graduation from an accredited registered nursing program.
Preferred Credentials:
Bachelor's Degree in Nursing.
Bachelors of Science (BS) in Nursing.
Unit Details:
Performs Utilization Review and Discharge Planning for assigned patients.
Analyzes medical records for medical necessity, level of care, and appropriate utilization of hospital services.
Provides assistance to patients and their families in planning discharge and aftercare.
Assesses needs, identifies resources to support discharge requirements, and develops formal discharge plans.
Initiates discharge planning and facilitates discharge teaching.
Ensures that each infant discharged from the NICU has follow-up by a primary care physician and, when appropriate, follow-up care in a specialized program for the high-risk infant.
NICU duties include determining whether an infant meets high-risk follow-up criteria, advising the parent/guardian of the process, making specialty clinic appointments, arranging physician-ordered durable medical equipment, facilitating home health follow-up, and referring qualifying infants to CCS.
Develops and implements discharge planning standards of care for the neonatal unit.
Pediatric duties include ensuring appropriate discharge information for each pediatric patient and communication with appropriate primary care physicians, community agencies, CCS programs, CCS Special Care Centers, and Regional Centers.
Pediatric discharge information is written in a culturally and linguistically appropriate manner and includes, but is not limited to, diagnosis, medications, follow-up appointments, and instructions for medical treatments to be administered at home.
Pediatric duties include teaching parents, legal guardians, or primary caregivers about the medical needs of the infant, child, or adolescent, including the use of necessary technology to support post-discharge care.
Coordinates appropriate multidisciplinary infant, pediatric, and/or adolescent healthcare team members in discharge planning conferences.
The assignment includes travel to ICU, DI, OR, etc.
Shift & Scheduling:
Primarily day shifts.
Floating Requirements:
Floating within the cluster/similar department is highly desirable.
