Additional Details:

Required Skills/Experience: 1 year experience needs to be Home Health. Wound care/management, foleys, PICC/Infusion, comfortable working with interdisciplinary team, central/PICC line dressing changes, de-accessing ports.

Required Credentials: Oasis Assessment

Unit/Facility Details: Parking: Dependent on patient housing

Shift & Scheduling:

• Mon-Friday, Holidays can be made up with weekend rotation or unpaid off. Holiday Expectations: None, can work weekend Sunday before or Saturday after if not taking unpaid holiday off. On Call, Weekend rotation option with Weekdays take back. Weekend rotation with Weekday take back.

Floating Requirements:

• Within 5 miles radius.
• Yes, see below for coverage territory.
• In rare instances, may overflow to zip codes 60617, 60619 60620, 60643, 60628 if primary area needs change.
• Shift times: 0800 until last patient is complete
• Schedule cycle: Monday – Friday, unless it’s their weekend to work. Will adjust schedule accordingly
