Additional Details:

Required Skills/Experience:

• Oasis experience is required.
• Experience with acute care, outpatient clinic, rehabilitation hospital, and skilled nursing facility settings.
• Orthopedic experience: cervicalgia, low back pain, post-op cervical surgery, post-op shoulder repair, post-op spinal surgery, spinal conditions, total hip replacement, and total knee replacement.
• Neurologic experience: cognitive disorders, multiple sclerosis, muscular dystrophy, Parkinson's disease, post-stroke rehabilitation, spinal cord injury, and traumatic brain injury.
• Home exercise program and patient/family education.

Preferred Skills/Experience:

• Home health experience preferred; travelers with no home health experience may still be considered.

Special Requests:

• First-time travelers are not accepted.
• This facility cannot accommodate 4x10s or 3x12s.
• Cannot be currently or recently employed with the network within the past 6 months; former employees must have left in good standing.

Unit Details:

• Home health physical therapy covering Medford and surrounding areas.
• May visit up to 6 patients per day; 30 productivity points standard.
• Clients are generally grouped to minimize drive and windshield time, but occasional travel radius may exceed 50 miles.
• Home health documentation includes OASIS. -Traveler is expected to have their own transportation. -Parking Cost: Free

Shift & Scheduling:

• No weekend requirement.
• No on-call requirement.
