Travel Allied (Physical Therapist) - Home Health

Additional Details:

Preferred Skills/Experience:

• Two years of clinical experience preferred.
• Home health experience preferred but not required.

Required Credentials:

• Reliable transportation, valid driver's license, and auto insurance required for home visits.

Unit Details:

• Conduct initial evaluations, progress notes, 30-day reassessments, and 60-day recertifications.
• Develop, implement, and update individualized plans of care aligned with physician orders and patient needs.
• Deliver skilled interventions and maintenance therapy programs.
• Directly supervise Physical Therapist Assistants and Home Health Aides.
• Prepare and submit OASIS assessments, recertifications, transfers, resumptions of care, and discharges.
• Apply ICD-10 coding for documentation.
• Care is provided in patients' homes and community settings.
• Work independently with minimal daily oversight.
