Additional Information
All positions at Providence Health & Services require that any and all lab items requested during onboarding be completed within 72 hours of signing a contract - no exceptions. This means candidates must be responsive and willing to go to a lab to complete health orders ASAP after signing. Failure to comply may lead to a pushed start or cancellation as assessed by the facility.
References must cover at least 1 full year of relevant experience in specialty within the past 3 years
You must be able to complete labs within 48 hours of signing.
ER profiles must include the size of the facility / # beds and the Trauma Level designation (if any)
Orientation for all Oregon facilities is in Portland, OR for RN, LPN and CNAs.
Required Skills/Experience:
The unit hires both rehabilitation and medical-surgical RNs.
Preferred Skills/Experience:
Previous Charge Experience: Preferred
Required Credentials:
Cannot have any hits/suspensions on license or any background hits from any point in the past.
Special Requests:
RTO must be listed at time of submission.
Cannot be currently or recently (within 6 months) employed with network - Must have left in good status, if former employee.
Must have a vehicle for this assignment.
Unit Details:
Adult patient population.
This is a post acute care inpatient rehab unit. The patients discharge from acute care and admit to our rehab unit. The patients have 3 hours of therapy per day (PT, OT, SLP) to rehab with a goal of discharging to home.
Patient population includes stroke, spinal cord injury, TBI, amputation, and debility.
Patient ratios are 1:4 on days and weekends and 1:5 on nights.
Shift & Scheduling:
No call.
Every other weekend.
Floating Requirements:
Will float between units per unit needs and based on the clinician's skill set.
May be required to float to other units and facilities to meet Providence Health & Services needs, including Milwaukie, Newberg, Portland, Hood River, Willamette Falls, Seaside, and Saint Vincent.
If floating consecutive shifts, the clinician will be additionally offered a hotel.
The facility will give as much notice as possible when floating to Seaside.

• AUTO OFFER - NO INTERVIEW: This is an auto-offer position. You will likely not interview with the facility prior to receiving an offer. However, an interview can be arranged upon request. All time off requests must be submitted at application. Scheduling requests must be worked out with the manager directly upon starting the assignment. Any offers for this job will require a clear "Yes" or "No" response from the nurse within 24hrs or the offer must be declined.
