Preferred Skills/Experience: Five years' hospice nursing experience preferred.
Required Credentials: Valid driver's license; must be at least 21 years of age or older. Current automobile insurance and reliable automobile required.
Unit Details: Coordinates and delivers comprehensive end-of-life care to terminally ill patients in accordance with hospice philosophy and regulatory guidelines. Manages a caseload of patients, conducts thorough assessments, develops individualized care plans, and provides skilled nursing interventions to address physical, emotional, and psychosocial needs. Provides symptom management, medication administration, pain and symptom control, wound care, and patient and family education. Serves as the primary case manager and point of contact for patients, families, physicians, social workers, chaplains, hospice aides, and the interdisciplinary hospice team. Typical caseload: 12-18 patients.
Shift & Scheduling: On-call rotation required for after-hours patient needs.
Floating Requirements: You may be required to float to Adventist Health locations within 30 miles.
