Unit Details:
- Home care case management with OASIS documentation.
- High-tech home health skills include IV use and maintenance, wound VAC, and complex wound care.
- Coordinates interdisciplinary care with physicians, providers, team members, vendors, physical therapy, and social work; supervises HCA services.
Shift & Scheduling:
- Tuesday-Saturday, 8am-4pm
- On-call rotation required; hospice coverage required when on-call.
- Holidays as required.
