Travel Nurse (RN) - Home Health

References must cover at least 1 full year of relevant experience in specialty within the past 3 years. ER profiles must include the size of the facility / # beds and the Trauma Level designation, if any. Orientation for all Oregon facilities is in Portland, OR for RN, LPN, and CNAs.

Required Skills/Experience: 1 year of skilled home health experience required.

Preferred Skills/Experience: Community hospital experience is preferred. Disease management, PICC lines, and catheter care and education preferred.

Required Credentials: Cannot have any hits/suspensions on license or any background hits from any point in the past.

Special Requests: RTO must be listed at time of submission. RTO restrictions: confirm with hiring manager. All positions at Providence Health & Services require that any and all lab items requested during onboarding be completed within 72 hours of signing a contract - no exceptions. This means candidates must be responsive and willing to go to a lab to complete health orders ASAP after signing. Failure to comply may lead to a pushed start or cancellation as assessed by the facility. You must be able to complete labs within 48 hours of signing. Cannot be currently or recently (within 6 months) employed with network. Must have left in good status if former employee.

Unit Details: Covers West Portland and surrounding areas. May visit up to 6 patients per day; 30 productivity points standard. Occasional maximum radius of roughly 60 miles (60-90 minute total drive time) per day. Adult and geriatric patient population. Uses Rover phone app for charting.

Shift & Scheduling: 4x10s and 3x12s cannot be accommodated. No weekend requirement. No call.

All time off requests must be submitted at application. Scheduling requests must be worked out with the manager directly upon starting the assignment.
